Initial 1-hour Assessment - What's involved?

Our occupational therapist meets you in your home, virtually, or a preferred location, and together you will: 

  • Review relevant medical history

  • Discuss areas of concern and goals

  • Complete a functional assessment

  • Discuss recommendations

  • Collaborate on a plan

  • Coordinate with stakeholders (i.e. family, healthcare providers, vendors, contractors, employer, etc.)

By the end of the session you will have been provided with actionable recommendations, resources and support to

increase your independence, comfort, and safety.
